Health & Rehabilitation Center: Rates

All residents must be admitted to and discharged from the Health and Rehabilitation Center (Skilled Nursing Facility) on the orders of a physician.

Life Lease Residents have a priority as stated in their Residency Agreement. Residents from the community are admitted to the center as space is available.

Fairhaven is a Medicaid and Medicare certified facility.
